There are many components to a comprehensive lymphedema treatment program.
EVALUATION
Evaluate to determine the appropriate course of treatment to take and to determine limitations an individual may have with lymphedema.
EDUCATION
Education is an essential component to ensure self management and control of lymphedema. Patients "modify" their activity level to accomodate for lymphedema. It is important to be knowledgeable about prevention of infections, recognizing onset, signs and symptoms of infections, and what steps to take if an infection is suspected. Proper skincare, elevation techniques, and nutrition also play important roles in successful self management.
MANUAL LYMPH DRAINAGE (MLD) or COMPLETE DECONGESTIVE PHYSIOTHERAPY (CDP)
These are special massage techniques consisting of a very gentle massage of the trunk and the limbs that stimulates the flow of lymphatic fluid through the system and back into the bloodstream. Lymphedema therapists are trained at a number of different lymphedema courses. They are more commonly licensed and practicing Physical Therapists or Occupational Therapists. The basic principles of treatment seem to be similar and effective, however the techniques may vary depending on where the therapist is trained.
COMPRESSION BANDAGING
Bandaging for lymphedema is done with low-stretch bandages which enhances the muscle pumping action of the limb, facilitates lymphatic flow, and reduces swelling. Bandages are worn during the night. The ReidSleeve® and CircAid® (nighttime, non-elastic support) arm and leg garments can be used in place of low-stretch bandages for home maintenance of extremity lymphedema.
THERAPEUTIC EXERCISE
Therapeutic exercise includes flexibility exercises and strengthening to enhance lymph flow.
COMPRESSION LEG STOCKINGS, ARM SLEEVES AND HAND GLOVES
Graduated medical compression (daytime, elastic support), such as Juzo and Jobst are worn during the day to maintain increased circulation and help prevent recurrent edema (swelling) and infection.
COMPRESSION DEVICE
Sequential, gradient compression therapy (the Lympha Press) can be used as a component of a comprehensive lymphedema treatment program. This device consists of ten to twenty four overlapping air compartments that inflate and deflate in a distal to proximal “milking” action. It mimics the lymphatic system. It is typically used in the evening for 1-2 hours while watching television or reading. |
